Bone Marrow Transplant Elective

Description:

This will be an off-site elective.  Fellows will spend 1-2 months at a Bone Marrow Transplantation Center.  During this time the fellow will become integrated in the BMT care team.  He/she will take call as directed by the supervising physician in accordance with duty hour rules for fellows/residents.

Goals and Objectives:

For a patient who is referred for consideration of BMT, the fellow will be able to propose a comprehensive plan of care and manage a the patient’s care.

Evaluaton:

At the end of the rotation, the fellow will be evaluated utilizing the locally adapted evaluation form available electronically on MEC.  This will be a composite evaluation completed by the director of the elective.  Additionally, the elective director will discuss the evaluation with the fellow. 

Competencies:

I. Patient Care

A.     The fellow will demonstrate an ability to care for patients undergoing bone marrow transplantation.  This will include having the ability to assess patients/donors and apply a rational plan of care.

II. Medical Knowledge

A.     The fellow will be able to discuss the indications for and complications of BMT.

B.     The fellow will be able to compare and contrast differences between autologous and allogeneic BMT.

C.     The fellow will be able to describe the pharmacology of different chemotherapeutic and immunosuppressive agents used in the treatment of patients undergoing BMT.

III. Practice Based Learning

A.     The fellow will plan a seminar/journal club related to the discipline of BMT.

B.     The fellow will examine the evidence for bone marrow transplantation in detail in at least one blood disorder and discuss with his/her supervising physician.

IV. Interpersonnal and Communication Skills

A.     The fellow will be able to communicate to referring physicians patients’ clinical status both in the form of a written report and through oral communication.

B.     The fellow will be able to explain clearly and in lay terms the indications for, process of, and complications of BMT to patients and their families.

V. Professionalism

A.     The fellow will treat all BMT staff with respect, appreciating that a team effort is required to care for patients.

VI. Systems Based Practice

A.     The fellow will understand the general mechanisms in place for the reimbursement by insurance companies to programs for the cost of BMT.

B.     The fellow will discuss the role of the National Marrow Donor Program in BMT practice.
