continuity clinic and ambulatory pediatrics

Continuity Clinic and Ambulatory Pediatric rotations are provided through institutional practices at CCMC, St. Francis Hospital, and the Burgdorf-Fleet Community Health Center. In addition, 13 community practices also offer continuity practice opportunities. The three institutional sites are located in Hartford, and provide care for under-served patients and families. University of Connecticut residents, along with dedicated and talented primary care faculty, provide primary care for 80% of Hartford's children. This allows residents to develop and apply their skills in child and family advocacy. Residents have ambulatory pediatric practice assignments during part of their ten rotation core ambulatory set of rotations.

Each entering PL-1 is assigned to an ambulatory practice that usually includes their Continuity Clinic, which meets one afternoon per week at their practice site. Residents also have the option of having their continuity experience at a community practice. These residents have one ambulatory rotation at their community site, and the opportunity to do the remaining ambulatory rotations in one of the institutional primary care sites. Residents who have both continuity experience and ambulatory experience at an institutional site, must have one ambulatory rotation in a community practice.

Throughout the three years of training, residents provide primary care to their own patient panel under the supervision of a dedicated continuity preceptor. The design of the continuity and ambulatory practice experience allows for a substantial, longitudinal component throughout the three years of training. Each resident will provide primary care for his or her own patient panel not only during ambulatory rotations, but also for a half day each week when on rotations away from the ambulatory site. Both scheduled well-child and acute care visits are seen at all ambulatory sites. Patients are assigned to provide an appropriate mix of healthy newborns and older children as well as those with chronic disease or multiple handicaps. Continuity Clinic responsibilities supersede all other clinical and administrative responsibilities, and as such, this clinic receives priority within the residency. The Continuity Curriculum and Practice Committee, composed of faculty and housestaff, oversees the functioning of this activity which includes: a formal curriculum, an observational evaluation component, regular conferences, and weekly one-on-one clinical teaching.
