Obtaining Patient Condition Reports
The most recent HIPAA (Health Insurance Portability and Accountability Act) guidelines went into effect in April 2003. It has helped to further protect the privacy of our patients and
has changed the way medical information about patients can be released.
In compliance with HIPAA and in accordance with American Hospital Association standards, the UConn Health Center
will release one-word condition reports. The reporter's query must include the patient's correct name.
Under HIPAA, patients have the right to "opt out" and block release of this information. When a patient "opts out," no information can be released about that patient, nor can it be
acknowledged that the person is a patient at the UConn Health Center.
From 8 a.m. to 5 p.m. Monday through Friday, reporters may call the Health Center’s Office of Communications at 860-679-4864 or 860-679-3914 to request a patient condition. During non-business hours,
call 860-679-2000 to reach the communications officer on call.
Patient Condition Definitions
- Good: Vital signs are stable and within normal limits. Patient is conscious and comfortable. Indicators are excellent.
- Fair: Vital signs are stable and within normal limits. Patient is conscious, but may be uncomfortable. Indicators are favorable.
- Serious: Vital signs may be unstable and are not within normal limits. Patient is acutely ill. Indicators are questionable.
- Critical: Vital signs are unstable and not within normal limits. Patient may be unconscious. Indicators are unfavorable.
- Undetermined: Patient awaiting physician assessment.
- Treated and Released: Received treatment but not admitted.
- Treated and Transferred: Received treatment. Transferred to a different facility.
- Deceased: Upon notification of family.
